1. You’ll receive a secure video link before your appointment.
2. Log in from a smartphone, tablet, or computer with a camera.
3. Ensure you are in a quiet, private space.
4. Have your medications ready to review.

Mobile / Home Visit Information
We offer house calls for patients who prefer care in the comfort of their home.
What to Expect
• A clinician arrives at your home at the scheduled time
• Bring any medications, equipment, or home medical records to review
• Home visits are ideal for:
• Limited mobility
• Post-surgical patients
• Pain management
• Complex chronic conditions
• IV therapy / antibiotic administration
Patient Portal Access

Medication Refill Guide
To ensure timely refills:
• Use the patient portal or call the office
• Allow 48–72 hours for processing
• Controlled substances may require a follow-up visit

We do not provide emergency services.
Call 911 or go to the nearest ER. After-hours:
• Leave a voicemail or portal message
• Messages will be returned the next business day
